This item is from The Legend of Zelda series and is typically awarded after a boss fight or after completing some other difficult task. In Smash, it heals 100% of damage, not to be confused with all of your damage. If your damage percentage is higher than 100%, you won’t heal all of your damage, only 100% of it.
